Outreach Reporting Form

 

Submit forms as projects are completed, but not later than August 1 of each year.  Please use one form per project.

 

Send your completed form(s) to the Carolinas Region Community Outreach Chair, Mary Mahaffey, 3620 Glashields Way, Apex, NC 27539-6886 or via e-mail to gottastitch@bellsouth.net.

 

The reporting form is provided in 2 versions.  The PDF version can be printed only.
