  • Sample proxy form, to authorize substitutes for a chapter president or region representative to vote at a Region Meeting.  Please note that the Carolinas Region does not have an official proxy form; the form here was used by one of the region's chapters and it is offered for optional use.  The form is available in both Word document and PDF formats.
